Knoxville Hospital & Clinics was recently awarded a Coverys Community Healthcare Foundation grant valued at $15,000.

The funds will be used to promote safer transitions of care from hospital to home and assist in the improvement of patient safety and readmission prevention. Coverys is a Community Healthcare Foundation that was established to support charitable organizations and endeavors who improve patient safety and care, promote healthy lifestyles, and support individuals and organizations that provide healthcare.

Director of Quality and Compliance at Knoxville Hospital and Clinics Elly Shaw tells KNIA/KRLS News the hospital is looking to reduce patients who are re-admitted to the hospital within 30 days of their previous admission. Shaw said that patients who have medication issues, at-home safety hazards, or monitoring chronic conditions can all contribute to another hospital stay.

Shaw went on to say that by utilizing these grant funds to provide additional services to help patients become accustomed to their new way of living, patients will find that readjusting to their home after a hospital stay will be much simpler and safer.
