A Knoxville woman was arrested Saturday on four felony charges and one aggravated misdemeanor charge.
Melisha Miller, 43, of Knoxville was arrested Saturday, April 15th for the charges of fraudulent practice in the 1st degree a class C felony, identity theft a class D felony, forgery a class D felony, wanton neglect of a dependent adult a class D felony, and tampering with records an aggravated misdemeanor. According to court documents the arrest stems from a period between December 2013 and October 2016 in which Miller is alleged to have knowingly and intentionally submitted documentation used in billing the state of Iowa for services she did not provide. Miller also allegedly forged the signature of a Medicaid member, and used the name and Medicaid identification number on 21 claims for targeted medical forms. Miller allegedly knowingly and intentionally denied access to a medical provider by refusing transportation in which she was contracted to provide, and also refused to provide a dependent adult with their prescription medication when it was requested. Miller allegedly created false records in the form of claim for targeted medical forms and consumer directed attendant care daily service records that documented services she did not provide. Miller was previously arrested in January 2017 and charged with dependent adult abuse by financial exploitation, a class D felony. Miller has a preliminary hearing date set for April 20th.
